Philadelphia Dog Licenses

If you have a dog that is four months old or older, it must be vaccinated for rabies and licensed. Not only does the dog license help us get your dog back to you, but it also but helps us track rabies vaccination status for the dogs in our county.

Complete the application by using the form below. If you have a copy of your pet’s proof of rabies please attach it. If you do not have proof of rabies please identify your veterinarian so that we may contact them. 

By Mail 

Please download the the Philadelphia Dog License application (pdf), complete the form and mail it in with your payment (cash or check) to 111 West Hunting Park Avenue, Philadelphia, PA 19140.
